Brief Job Description: The Clinical Supervisor/Nursing Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor/Nursing Supervisor is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. During the initial start up period, we will be seeking Medicare accreditation. This process may take up to 12 months. During this time the work load will be minimal, approximately 5 hours a month. The estimated patient census during this time will be 2 – 3 active patients per month. Each patient with a CHHA assigned requires a supervisory visit every 14 days. Each LVN requires a monthly supervisory visit. After a Medicare Provider ID is obtained, the work load will increase as the census grows. The founding medical director and administrator has over 12 years experience in hospice care and will support and guide this position during the entire process as needed.
Qualifications: A nurse supervisor shall have a current RN license, with at least two (2) years of hospice, home health, or community health experience in the last five (5) years.
